MSI Australia ensures client confidentiality and support via a range of processes and policies Australia-wide.
All MSI Australia clients are in the care of specialised practitioners who work hard to support them with care and any required information to ensure their health journey is comfortable and confidential.
Accessing client information
Clients
Complete the Release of Information and Authorisation Form and provide documentation to validate the request. Details of these requirements are located on the form and below for your reference.
- Copy of Photo Identification e.g. driver’s license or passport
- Copy of Health Care Card or other documentation for exemption (see criteria for exemption).
External health care providers
Your request must be in writing and on your organisation’s letterhead or from the business email address.

Third Parties
Your request must be in writing, accompanied by client’s authority for the release of information.
Alternatively, complete the Release of Information and Authorisation Form.
Request for information
Once we have received your valid request, you will receive communications acknowledging your request and what the cost of processing will be. Full payment is required prior to the release of information. You can expect to receive your response 28 days from the date we received your completed request. Timeframes may be extended for certain requests, or where more information is required.
